Family Caregiver Support Program

Life changed on December 2, 2018, when Scott was involved in a hit-and-run automobile accident, resulting in a severe head injury that led to permanent brain damage and swelling. Following 28 days on a ventilator at Harborview Medical Center, Scott became permanently bedbound and now requires specialized equipment for transfers from his bed to a custom wheelchair. Maria, Scotts wife and caregiver, has been a Homage client since 2018, and after her cancer diagnosis while caring for Scott, she reached out to Homage for help.

Caregiving is a stressful and often unrecognized role for many friends and family members caring for someone with dementia, other chronic illnesses, or disability. The Family Caregiver Support Program helps unpaid caregivers (18 and over) with free consultations that connect them with community resources and support that best fits their situation. 

Consultation includes:

  • Caregiver assessments and Care Planning through the Tailored Caregiver Assessment and Referral Theory (TCARE), which provides meaningful evaluations of the challenges, stresses, and satisfactions of caregiving. 
  • Information and emotional support
  • Referrals to support groups, respite, and other options.
  • Workshops on reducing stress, increasing self-care, improving communication, managing emotions and planning for the future.
  • Caregiver-focused events

The first step in the caregiver screening and assessment process is filling out a TCARE® Personal Family Caregiver Survey. The survey asks you to reflect on your experiences and is used to help you and the Family Caregiver Support Program better understand your situation to help locate the resources and support that meet your unique needs.
