When you connect with Homage, you’re paired with a Care Coordination Specialist who walks alongside you. They listen, help you identify what you need, and work to remove barriers so you don’t have to keep retelling your story to multiple providers. Our goal is simple: to ensure you receive care and support that is timely, coordinated, and centered around you.
