Chronic Disease Self-Management Workshop
Living with a chronic health condition can feel overwhelming, but you don’t have to manage it alone. Our Chronic Disease Self-Management (CDSM) Workshop is a six-week, evidence-based program designed to help adults build the skills and confidence needed to better manage ongoing health conditions while maintaining independence and quality of life. This nationally recognized program was developed at Stanford University and is widely used in community and healthcare settings to support people living with conditions such as diabetes, arthritis, heart disease, chronic pain, lung disease, high blood pressure, and other long-term health concerns.
During the six interactive sessions, participants learn practical strategies that can be used right away in daily life. Topics include:
- Techniques for managing pain, fatigue, and frustration
- Safe and effective ways to stay physically active
- Making healthier food choices that support long-term wellness
- Relaxation and stress-reduction techniques
- Setting realistic goals and creating action plans
- Communicating more effectively with doctors, pharmacists, and other health providers
- Problem-solving skills to handle new challenges as they arise
- Building confidence to take an active role in your own health care
Workshops are led by trained Homage facilitators in a supportive, small-group setting where participants can share experiences, learn from one another, and practice new skills together. The program is designed for adults with any chronic condition, as well as caregivers who support someone living with a long-term illness. No special fitness level or prior experience is required.

Space is limited, and advance registration is required. Participants are encouraged to attend all six sessions to receive the full benefit of the program.
Peer Support Groups
Learn skills for emotional wellness while making new supportive relationships in the community! The groups are designed to provide ongoing support for Snohomish County adults 60+. Past groups have focused on depression support, visual art, writing, healthy relationships, and more.
