Family Caregiver Support Program

Caregiving is a stressful and often unrecognized role for many friends and family members caring for someone with dementia, other chronic illness, or disability. The Family Caregiver Support Program helps unpaid caregivers (18 and over) with free consultations that connect them with community resources and support that best fits their situation.
Consultation includes:
- Caregiver assessments and Care Planning through the Tailored Caregiver Assessment and Referral Theory (TCARE), which provides meaningful evaluations of the challenges, stresses, and satisfactions of caregiving.
- Information and emotional support
- Referrals to support groups, respite, and other options.
- Workshops on reducing stress, increasing self-care, improving communication, managing emotions and planning for the future.
- Caregiver-focused events
The First Step
The TCARE Personal Family Caregiver Survey (click here to take the survey online) asks you to reflect on your caregiving experiences and is used to help you and the Family Caregiver Specialist better understand your current situation. Once a survey is submitted, a Specialist will contact you to discuss your results and explore the resources and options that best fit your needs.
